Home-based rehabilitation after stroke is increasingly important, as pressure on hospital services rise and recovery occurs mostly at home. Enriching the home (Environmental Enrichment, EE) represents a potential strategy to support rehabilitation by providing continuous task- and context-specific stimulation that promotes functional recovery through frequent and exploratory use of the affected arm. Exploratory behaviour - self-initiated, varied and adaptive movements - is an important driver of...

Venous leg ulcers (VLUs) are a chronic and often debilitating condition, affecting a significant number of individuals worldwide. One of the most distressing aspects of VLUs is the persistent pain they can cause, which impacts quality of life, mobility and overall wellbeing. Despite advances in wound care, VLU-related pain remains under-recognised and poorly managed. Pain in VLUs is complex and typically includes nociceptive pain from tissue damage, neuropathic pain due to nerve involvement, and...

CONCLUSIONS: This prospective observational study found that different ventilation strategies influenced catheter-tissue interaction and procedural metrics during pediatric RF ablation. Notably, HFLTV ventilation was linked to higher contact force metrics. Additional research is necessary to verify these results.

Glutamate transporters are essential for maintaining CNS homeostasis by clearing extracellular glutamate following synaptic transmission. Dysregulation of these transporters contributes to glutamate-mediated excitotoxicity across numerous neurological disorders, including ischemic stroke, underscoring their potential as therapeutic targets. However, the regulatory response of these transporters following ischemic insult remains poorly defined. In this study, using a model of oxygen-glucose...

Stroke occupies an important position in the global disease burden, but the combined effect of heavy metal exposure and the inflammatory marker C-reactive protein (CRP) on stroke remains unclear. Based on the National Health and Nutrition Examination Survey, this study explores the association between blood lead, blood cadmium, blood mercury, CRP, and the risk of stroke. A total of 8297 American adults aged 20 years and older were included in the National Health and Nutrition Examination Survey....

An integrated workflow for nonischemic dilated cardiomyopathy (DCM) has yet to be developed. CONCLUSIONS: In this national in-home echocardiography study, VHD was common among older adults, with important age-related and valve-specific patterns. PREVUE-VALVE establishes the feasibility of large-scale decentralized cardiovascular imaging studies and provides a contemporary foundation for clinical and policy planning related to the burden of VHD. CONCLUSIONS: ANP was identified as an independent predictor of PAF detection in patients with ESUS, with strong ROC performance. Given its atrial origin and very short half-life, ANP may serve as an atrium-specific biomarker reflecting acute atrial load, potentially enabling earlier identification of latent PAF in the acute phase.

Cell signaling regulates cellular physiological functions, including the cell cycle, survival, immune response, gene expression, and protein-protein interactions. Within this complex system and its intrinsic mechanisms, non-receptor tyrosine kinases (NRTKs) play a crucial role in signaling in the cellular cytoplasmic compartments. Owing to the involvement of NRTKs in the signaling cascade, they have made a significant contribution to the manifestation of etiologically complex diseases such as...

Clopidogrel therapy is recommended to prevent recurrent ischemic stroke. However, clopidogrel resistance exists. This study examines which enzymes are involved in clopidogrel resistance. The study group includes 402 neurology patients who had experienced a recurrent ischemic stroke while receiving clopidogrel monotherapy. Patients were genotyped for CYP2C19, CYP2C9, CYP2B6, CYP3A4, CYP3A5, CYP1A2, ABCB1 and CES1A1. Phenotype frequencies were compared with those of a Global(G), European(E) and a...

Despite substantial therapeutic advances in multiple myeloma (MM), extramedullary disease (EMD) remains an aggressive subtype associated with poor prognosis, for which consensus on management is lacking and dedicated clinical trials are scarce. We conducted a retrospective single-center analysis of 86 patients with radiologically confirmed EMD, defined as soft-tissue involvement without bone contiguity (data cut-off September 30, 2024). Dysregulated intracellular lipid metabolism emerged as a driver of heart failure (HF), a leading cause of death worldwide that is frequently caused by dilated cardiomyopathy (DCM). Yet, how defective lipid signaling destabilizes failing cardiomyocytes (CMs) at the molecular level remains elusive. We utilized induced pluripotent stem cell-derived cardiomyocytes (iPSC-CMs), patient-derived heart tissue, and living adult CMs to elucidate lipid-dependent endoplasmic reticulum (ER) dysregulation in...

Titin, the largest human protein, forms the elastic sarcomeric backbone, providing passive stiffness and length-dependent activation in cardiomyocytes. Whereas titin mutations cause inherited cardiomyopathies, ischemic and chemotherapy-induced injury also provoke proteolytic cleavage of titin's elastic segment. However, the effects of acute titin stiffness loss remain unknown. Here we develop a knock-in mouse enabling in vivo cleavage of cardiac titin springs and use multimodal analysis (cardiac...

Pyroptosis, as an inflammatory type of regulated cell death, is associated with the pathogenesis of various inflammatory diseases. Targeted therapy for pyroptosis has shown promise in multiple preclinical models of neurological injury and disorders. Stroke is one of the leading causes of morbidity and mortality worldwide and the top cause of disease-related death in China. Although the pyroptosis signaling pathway has been studied in cerebral ischemic diseases, its pathophysiological mechanisms...

Vascular cognitive impairment (VCI) comprises cognitive dysfunction caused by vascular pathology and is associated with various forms of dementia. VCI is typically caused by chronic cerebral hypoperfusion (CCH), which is accompanied by oxidative stress, central nervous system inflammation, microglial activation, and white matter injury. Pacemakers are implantable cardiac devices used in the treatment of bradycardia and heart failure. They monitor electrical signals in the heart and deliver electrical stimuli to cause contraction in the targeted heart chamber. Permanent pacemaker therapy is recommended in patients with symptomatic bradycardia or asymptomatic infranodal atrioventricular block to improve symptoms and quality of life. Biventricular pacemakers can also be used in cardiac resynchronization therapy, which corrects...

BACKGROUND: Antipsychotic use in people living with dementia has been linked to serious adverse outcomes. Guidelines recommend limiting antipsychotic treatment to short-term use (<12 weeks), followed by tapering and cessation. However, antipsychotic treatment in routine practice often exceeds the recommended duration. The effect of discontinuing antipsychotics on reducing adverse outcomes in practice remains unclear. We aimed to use linked primary and secondary care data in England to...

Background Ischemic lesion net water uptake (NWU), a CT biomarker of cerebral edema, has been linked to poor outcomes in patients with acute ischemic stroke (AIS) due to large vessel occlusion (LVO).
