Sarcoidosis, Pumonary Pubmed Results

Pulmonary sarcoidosis has a heterogeneous clinical course, with a subset of patients progressing to chronic disease and fibrosis despite first-line corticosteroid therapy. The mechanisms driving disease progression are poorly understood, although limited evidence suggests a role for neutrophils. The present study aimed to characterise neutrophils and neutrophil effector molecules in progressive pulmonary sarcoidosis. We first performed in silico transcriptomic analyses to evaluate neutrophil...

INTRODUCTION: Sarcoidosis is a heterogeneous granulomatous disease with highly variable clinical trajectories, yet no validated biomarkers exist to distinguish progressive sarcoidosis (P-sarcoidosis) from non-progressive disease (NP-sarcoidosis). This lack of tractable biomarkers limits early risk stratification and impedes therapeutic decision-making. Preliminary data from our group suggest that P-sarcoidosis and NP-sarcoidosis may be differentiated by blood-derived and peripheral blood...

INTRODUCTION: Sarcoidosis is a multisystem disorder with variable presentation and disease course. Diagnosis requires the exclusion of other causes of granulomatous inflammation. Current clinical management is often fraught with diagnostic uncertainy and the lack of tools to predict pulmonary disease progression. To address these challenges, we designed a study using data from bronchoalveolar lavage (BAL) fluid and cells to develop diagnostic and prognostic tools in patients with pulmonary...

The clinical presentation and both the nature and severity of organ involvement are highly heterogeneous and are influenced by ethnic, genetic, and geographic factors. Establishing the diagnosis can be challenging due to the wide spectrum of clinical manifestations. Careful assessment of the indication for pharmacological treatment, as well as the preferred agent, is of paramount importance in every patient to optimize management and to minimize the risk of potentially serious adverse effects....

CONCLUSIONS: A concerning upward trend in incidence, DALYs, and deaths related to ILD&PS was observed in the global population aged 55 years and older, particularly among females. To our knowledge, this is the first study to comprehensively analyze the burden of ILD&PS in this age group from 1990 to 2021. Our findings on epidemiological trends and their variations across geography, SDI, age, and sex can inform policy-makers in designing targeted strategies to mitigate the anticipated rise in...

Sarcoidosis is a systemic inflammatory disease characterized by heterogeneous organ involvement and an unpredictable clinical course. Although clinical assessment often focuses on objective measures of disease activity, many patients experience persistent symptoms that substantially impair their quality of life. Fatigue, cognitive disorders, dyspnea and pain are common and often markedly underestimated, particularly when objective disease activity appears limited. These symptoms may...

CONCLUSION: Pulmonary sarcoidosis is associated with occupational silica, pesticides, WTC dust and mould. Future research should prioritise gene-environment interactions and granuloma mineralogy to refine preventive strategies and disease management.

INTRODUCTION: Sarcoidosis frequently involves intrathoracic lymph nodes and lung parenchyma and requires histological confirmation for diagnosis. Although endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is recommended for obtaining mediastinal samples, its diagnostic yield frequently necessitates additional transbronchial lung biopsy (TBLB) and/or endobronchial biopsy (EBB). EBUS transbronchial mediastinal cryobiopsy (EBUS-TBMC) can yield larger, well-preserved...

CONCLUSIONS: DCP-VD is selectively reduced in sarcoidosis, suggesting early subclinical microvascular compromise, particularly in extrapulmonary and ocular phenotypes. OCTA may serve as a valuable non-invasive tool for detecting subtle retinal microvascular changes before clinical signs appear. Further longitudinal studies are warranted to assess the prognostic implications of these findings.

We evaluated 12 workers with documented exposure to respirable crystalline silica who were referred to a tertiary care center due to clinical suspicion of silicosis, sarcoidosis, or silicosarcoidosis. Although silica exposure is a well-established risk factor for silicosis and has been associated with autoimmune diseases, mycobacterial infections, and lung cancer, growing evidence also suggests a link with sarcoidosis, creating important diagnostic and therapeutic challenges. All patients...

We report here on a patient with a rare pulmonary cause of third-degree AV block who was initially referred to us for further investigation of generalized lymphadenopathy. He had a history of third-degree AV block in December 2023, where etiology could not be determined even after extensive diagnostic workup. Following a pulmonary embolism that occurred afterwards, incidental findings of unclear generalized lymphadenopathy as well as splenic and liver lesions were seen on CT scan. For further...

BACKGROUND: Sarcoidosis is an uncommon systemic immune-mediated inflammatory disorder associated with noncaseating granulomas, most often in the lungs and less frequently in other visceral sites and the brain, skin, and lymph nodes.

Sarcoidosis is a complex multisystem disease whose impact on daily functioning and work ability is often underestimated. Traditional medical assessments tend to focus primarily on objective parameters such as lung function, while difficult-to-measure yet disabling symptoms - such as fatigue, cognitive impairment, and pain - are frequently overlooked. This can lead to incomplete and sometimes unjust assessments of work capacity. A holistic approach that integrates clinical findings with patient...
