Community Rehabilitation Service

 The Community Rehabilitation Service (CRS ) is based at The Heart of Hounslow Centre for Health in Bath Road Hounslow.

The team has physiotherapists, occupational therapists and rehabilitation assistants who provide rehabilitation at home to people who have lost their ability to be totally independent.

The team can offer:

·        a group or individual programme to make you more independent and help you to manage your condition

·        Support and information based on up-to-date specialist knowledge, skills and experience

 Depending on your needs, sessions are held at the Heart of Hounslow Centre for Health, in your home, your workplace or day centre.

Referral is through a G.P. or other healthcare professional.

Adult Speech and Language Therapy Service

The Adult Speech and Language Therapy Service provides assessment, advice, treatment, education and support for people with acquired swallowing and communication difficulties including understanding, speaking, reading and writing. The service works closely with family and carers to provide information, advice and support.

Swallowing and voice problems need referral from a health care professional but people can self-refer for communication problems.

People may be seen for individual sessions and/or group sessions.

The groups offered include groups to practice language skills and consolidate the use of communication strategies, groups to practice advanced language skills, and groups to build up confidence and provide mutual support.

 My Word is an aphasia support group which meets to enjoy others company, laugh and share experiences. Anyone living with aphasia in the borough of Hounslow is welcome to attend.

My Word is currently being held at Isleworth Working Men’s Club 109 St Johns Road off Twickenham Road TW7 6PN
