Support Services for Carers

If you provide regular and substantial care for a partner, friend or relative you are entitled to a carers assessment.

 If someone is being discharged from hospital and you will be caring for them you should ask for a carers assessment from a social worker in the hospital before they leave.

Help for carers can include vouchers to enable carers to take a break from caring, information and advice, training, the provision of respite and practical help at home. The carers emergency card gives peace of mind by making sure that, if an emergency happened, arrangements for alternative planned care can be put in place.
